Key Request Form

Refer to Section III Office Policies and Procedures, page 4 of the Policy and Procedures Manual. Anyone desiring a key to church property that is to be kept in their possession must receive permission by vote of the church based of a recommendation of the Physical Operations committee. A written request stating the need for such key shall be submitted to the church office for consideration by said committee. Please write your request below:
